There was general agreement May 22 at a hearing on blowout preventers: The unthinkable did happen at the Macondo well blowout on April 20, 2010: well control was lost; the blowout preventer, or BOP, failed; and as a result lives were lost and the nation’s worst on-water oil spill occurred.
